New Patients
Before you arrive for your first visit here are a few helpful hints:
- Please arrive 20 minutes before your first scheduled appointment to complete necessary paperwork and have x-rays taken. If you prefer, you may download the forms and bring the completed forms with you to save time.
- Please remember to bring your previous x-rays or MRI films and any reports. Your doctor can not complete the evaluation without the actual images for review (reports alone are not sufficient). We prefer, when possible, to have films provided on a computer disc (CD).
- If you have previously had surgery you will be requested to obtain copies of the operative report and any pictures from surgery to assist us in treating your problem. We can not see patients within 6 months of an operation without a referral from the original surgeon. We are happy to provide a 2nd opinion regarding your diagnosis or treatment, but please understand that often many different approaches to treatment exist and medical opinions do sometimes differ.
- In order to facilitate your examination, please adhere to the following: For shoulder or elbow problems please wear a tank top. For hip, knee, or ankle problems please wear loose "conditioning" shorts. We will provide you paper garments to change into if necessary.
- Please remember to bring your co-pay at the time of service. We accept cash, check and credit card.
- We are unable to accept 3rd party billing, liens, or legal claims at this time.
- We are pleased you have selected us to assist you with your healthcare needs. We look forward to meeting you.
